copying the form go to the demo dental account go to marketing > form builder click on the form, select copy next, rename your form to suite your new campaign and select the client the new form should be placed in modifying form options sticky contacts after this, go back to the client you are setting up the campaign for and go to marketing > form builder > select new form go to options and make sure "sticky contact" is selected modify on submit url next, you need to update the "on submit" option to send the completed form user to the thank you page of your funnel integrating form into funnel its important to note that you must use the funnel element "form" and not embed the iframe the funnel element will pass the form data to the next page while the iframe will not make sure you select the right form for the campaign from the dropdown in the form settings save changes and you're done!
